I've checked out lots of ideas online and since I still do not know if I'll be having a new niece or a new nephew, this is a combination of ideas for both genders.
Here is a clever Invitation Featuring the Ultrasound Photo. I love this idea!

Here is a super neat idea for creating a Baby Shower Tree as a centerpiece at the shower. You can hang anything from mini shower gifts, party prizes, ultrasound photos, baby photos of the parents-to-be or little ornaments on it.

Here's something clever to do for the food table at a shower. It's a "Watermelon Baby Fruit Salad."
These Baby Food Jar Party Favors get my vote for creativity! Make a label unique to your shower and fill with candy!
There's a couple ideas for using a clothesline that I would like to share. The first is having guests bring an unwrapped outfit (as part of their gift to the mom-to-be) to hang on a clothesline at the shower. Like this one at this Vintage-inspired Baby Shower:
The second idea involves using a clothesline to hang cut-outs that match your shower theme and colors. Like this beautiful Baby Shower Clothesline.
I, also, really love the idea of asking guests to come with a white onesie or plain bib that they decorated themselves. A bib or onesie can be sent along with the invite (yes, a couple extra bucks for extra postage for each guest ... maybe more-feasible for a smaller guest-list) or this suggestion can be made on the invitation (guests can choose to buy one on their own and decorate ... or not). The personalized bibs or onesies can be displayed on a clothesline at the shower like the ones in the above photos. Think along the lines of "I <3 My Auntie" or "Grandma's Little Princess." Mom and dad can even photograph the baby in each onesie and attach the photo with their thank you cards specific to each personalized onesie or bib.
